Grant Application Form

Apply for financial assistance for your retired service animal. Please complete all sections and attach required documents.

Please complete this form in full. Incomplete applications or missing attachments will delay processing.

Your animal must have been registered with the NFRSA for 30 days before a grant can be considered, unless this is a medical emergency. Invoices must be dated within the last 30 days. The NFRSA does not backdate veterinary bills beyond 30 days except in emergencies.

  • I hold a UK registered bank account and can accept bank transfers.
  • No relevant insurance / third-party claim applies to these costs.
  • I have not claimed through any third party for these costs.
  • I have not applied to any other organisation for the same costs.
  • I understand grant levels by length of service (30% / 50% / 90%) are subject to available funds.
